Usefulness of Enzyme Gel Test for Unexpected Antibody

Abstract
Unexpected antibodies related with hemolytic transfusion reaction show different reactivites according to temperature or enzyme treatment, Therefore, combination of various methods, such as 4"c, LISS/Coombs gel and enzyme gel, is necessary to accurately identify the antibody. But, most laboratories perform the antibody screening and identification test only with LISSICoombs gel due to low medical fee. In our laboratory, enzyme gel has been added when the action of LISSICoombs gel is indefinite since Jun 2007. In this study, the frequency of unexpected antibodies identified after addition of enzyme gel were evaluated to provide the guideline of unexpected antibody test with various methods. From Jun 2007 to Jun 2009, antibody screening was performed with 26,986 sera of transfusion candidates using LISS/Coombs gel (DiaMed, Cressier sur Morat, Switzerland) and antibody identification was performed with sera showing positive results in screening test, Also, enzyme gel was added when the reaction of the LISS/coombs gel in antibody identification was indefinite. As a result, 286 (1.1 %) of 26,986 sera showed positive screening results and antibody identification test was performed with them. Enzyme gel was added in 132 (46.2%) sera, Additional antibodies were identified with 93 (70.5%) sera in enzyme gel and 44 of them contained clinically significant antibodies such as anti-D, anti-E or anti-C. In conclusion, clinically significant antibodies were detected in 33.3% of cases that enzyme gel test was added. Continuous effort is needed to perform the antibody screening and identification test with various methods in all transfusion candidates with posltive antibody screening results for safe transfusion.
